1. The wife of his cousin
2. The cousin’s stepsister
3. The stepsisters in-law
Would lead you to beleave that the person was a stranger. About as real as meating your best friend on a random gaming site.

Ok, the way I read it is:
The woman that saw the picture is married to the narrators cousins wife’s, sister from another mother(or father or both), biological brothers wife.
